If using a differential with an A-arm setup, where should the relationship of the inner flange/CV cup on the diff be in regard to the lower A-arm pivot point? Should they line up, stick outward past the pivot point, or inward of the pivot point?

The closer you can get the center of the cv joints to be in line between the suspension pivot points (upper and lower) the less plunge you'll have.
